The Accidental Death and
Disablement Benefits provides the Cardholder/Beneficiary
a lump sum cash benefit of P200,000.00
if the insured Cardholder figures in an
accident which resulted to any of the
following conditions: |

a. |
death within 180
days after occurrence of the accident |
b. |
total and permanent
disability preventing the Cardholder
insured from engaging in any occupation
or employment as source of income |
c. |
total and permanent
loss of sight in both eyes |
d. |
total pysical
severance of any two or combination
of limbs, hands, arms, or legs. |

| 2. |
The
Accident Death and Disablement Benefit
shall be effective starting from 12:01
Noon, Manila Standard Time of the date
the Insurance Company receives the Prudentialife
Wellcare Cardholder data Return (CDR)
until 12:00 Noon, Manila Standard Time
of the Validity specified on the face
of the Prudentialife Wellcare Card, and
provided that the insured Cardholder has
not reached the age of 65. |

The Accident
Death and Disablement Benefit shall not
extend to cover loss or disablement caused
by the following: |
| |
a. |
air travel except
as a passenger in a fully licensed
passenger aircraft |
b. |
motorcycling |
c. |
parachuting, hang
gliding, mountaineering, underwater
activities, or any kind of race
or trial of speed and reliability |
d. |
war, invasion,
hostilities, terrorism, riot or
strike |
e. |
suicide or intentional
self injured |
f. |
pregnancy, childbirth,
or pre-existing physical or mental
defect infirmity |
g. |
influence of alcohol
or a drug taken without medical
prescription |
